in order to advance within the Apprenticeship Program, it is important to accurately record your hours each month. All hours MUST be logged on the last day of the month. The month at the top of the form will change the first day of the following month. If you do not log your hours by the end of the month you must write to the Training Director to request your hours be added for the month prior.
